Rotator Cuff Surgery Recovery, Weeks 3 to 6: What’s Normal
Your crew is on the job without you. You’re in the sling, still waking in the chair, wondering if it stays like this.
Bad nights at week 3 are common, and the repair is still far from full strength. After the sling weeks, you usually start moving the arm yourself, and desk or hands-only work often comes back around now.
What’s normal now
- Bad nights at week 3 are common. They’re a stage, not a verdict. Source
- Around 6 weeks, in one small study of 48 people, night pain came less often than before surgery. Source
- At 6 weeks, one NHS physical therapy guideline puts the repair at a fifth to a third of normal strength. It cites no source. Source
- Leaflets say to stay off the operated side at night. Two NHS leaflets say for 6 weeks. Source
- Desk work often comes back in about 3 to 8 weeks. Hands-only tasks, elbow at your side, often from about 6 weeks, with no lifting that arm. Source

Questions to bring to your surgeon or PT
- When can the sling come off? Most leaflets say about 4 to 6 weeks, but you set my date. Source
- Can I drive yet, and when can I reach behind me or work overhead? Source
- What’s off-limits for now, and what can I still train, like legs or seated work? Source
- What should make me call you before my next visit? Source
Don’t wait for your next visit
- A temperature of 101°F (38.3°C) or higher. Source
- Numbness or tingling in your fingers or hand, or a hand that looks darker or feels cool. Source
- Swelling in your arm, or pain that keeps building or that your medicine doesn’t touch. Source
- A snap after a fall or a jerk on the arm, then sudden weakness. A repaired tendon can tear again. Source
- Throbbing pain or swelling in one calf, thigh or arm. With chest pain or shortness of breath, call 911. Source
Chest pain or trouble breathing: call 911.
