ACL Surgery Recovery, Before Surgery: What to Plan For
Surgery is on the calendar. Your foreman wants a return date, and you’re wondering who carries your load until you’re back.
How long you’re off depends on how heavy your job is: one insurer’s guide puts heavy work like a builder’s at 10 to 16 weeks for a full return. Most workers in the studies got back, and an early return on modified duties is encouraged when appropriate.
What’s normal now
- If your job is heavy, plan in months, not weeks. Source
- One insurer’s guide for a full return: light work 4 to 8 weeks, medium 8 to 10, heavy 10 to 16, very heavy 16 to 30. Source
- In a Finnish study, manual workers were off about 11 weeks on average after ACL surgery. Source
- In both bigger worker studies, 9 in 10 or more got back to work. Manual and knee-demanding jobs took longer. Source
- Graft type matters if you kneel: in New Zealand’s registry, 21.3% with a kneecap-tendon graft had severe kneeling trouble at two years, versus 9.4% with hamstring. Source
What usually comes next
- New Zealand’s ACC says an early return on modified duties or hours should be encouraged, when appropriate. Source
- One NHS physical therapy team says light duties may be possible after 2 weeks, though you’ll likely still be on crutches. Source
- Plan rides: in one brief report after ACL surgery, braking matched healthy drivers by about 2 weeks for a left knee, about 6 for a right. Source
Questions to bring to your surgeon or PT
- Which graft will I get, and how does that fit my job and the kneeling I do? Source
- Can we set dates now for surgery, my first check-in and a target return to limited duty? Source
- What will my lifting limit be: pounds, distance, and stairs or not? Source
- Can I train my other leg hard, and are there exercises to avoid because of balance or crutches? Source

- List every duty and sort it into three buckets: only you, you with changes, and someone else. Name a lead for each handed-off task and set review dates. Source
